At Bolton & Company, we understand the risk and time sensitivity inherent to businesses. As part of our continuing efforts to issue Certificates of Insurance promptly and accurately, we are providing a simplified process for our clients.

Once completed, you may submit the form electronically. The request may also be printed and submitted to Elizabeth Foster, AIS, Certificate Analyst, by fax at (626) 405-2060 or by email to CertRequest@boltonco.com.

Our turnaround time is approximately 3 hours for rush certificates and 24 hours for all others.

Please note that coverage cannot be added, changed or deleted through this process and must be submitted to and confirmed by a member of our staff.
